Any Audiology clinic can take ear molds, and most are distributors for various brands of ear plug products. You can also go to a clinic to get molds taken ($30-$50) and then send them off yourself to whatever company you want to have plugs made. I had molds taken at the clinic at 144th & Center and then sent them off to Westone in Colorado and had a couple sets of ear plugs and custom headphones made from them.
